Human and Organisational Performance (HOP) is a contemporary safety philosophy that recognises human error as a symptom of deeper system issues, not the root cause of failure.  

Rather than focusing on blame or rule enforcement, HOP encourages organisations to learn from variability in work, understand how tasks are actually performed (“Work as Done”), and design systems that are resilient to error.
